A Health Information Exchange (HIE) is the electronic transmission of health-related information among care providers, communities, vendors and other relevant organizations—all according to nationally recognized standards.
Goal: to facilitate access to and retrieval of clinical data to provide safer, more effective patient-centered care.
What Are Personal Health Records?
The mobile-optimized Personal Health Record (PHR) is a complete health picture for an individual – from physician orders and encounters, complete problem list, dispensed medications and lab results, and insurance records.
The PHR offers allows individuals to develop a formalized and dynamic care team with defined roles. Invited care team members can view the personal health record, receive and respond to tailored and role-based encounter alerts, and interact with the individual in real-time.
Is Simply Connect Secure?
Yes. All personal health information is protected by the Health Insurance Portability and Accountability Act (HIPPA). This privacy rule gives every patient certain rights and sets limits on how the patient's information can be shared with others.
We do not store patient medical records. Instead, we provide a secure network for patients' health providers to share the information that they already store. Our sophisticated, HIPAA-approved data encryption ensures this information stays completely secure during transmission across our network.
How Do I Participate?
The solution facilitates the exchange of patient health information with EHRs that can produce and transmit/receive a Continuity of Care Document and enable bidirectional exchange using eHealth Exchange standards and specifications.
Simply Connect allows organizations to exchange e-health information with other organizations outside their local HIE by onboarding to eHealth Exchange, a national health information exchange made up of federal agencies and non-federal organizations that share patient health information using the same standards and specifications.
