Simply Connect has pioneered a proprietary application within the care management solution, allowing individuals to formalize a unified care team within their Personal Health Record. This enables individuals to give members of their care team visibility into their Personal Health Record. Other members of the care team can receive and respond to tailored and role-based encounter alerts, and interact with the individual in real-time.
Personal Health Record (PHR)
The mobile-optimized PHR is simply a complete health picture for an individual – from physician orders, complete problem list, vitals and lab results to dispensed medications and insurance records. The PHR allows individuals to develop a formalized and dynamic care team with defined roles. Invited care team members can gain visibility into the personal health record, receive and respond to tailored role-based encounter alerts, and interact in real-time.
Getting real-time notifications to the right people with the right information is critical to any care provider’s ability to improve outcomes and costs.
